Covid Reporting
Labette Community College continues to monitor COVID-19. We are requiring members of our college community to fill out this form if they have tested positive for COVID-19, tested negative for COVID-19, are awaiting the results of a COVID-19 test, or believe they have been exposed to somebody who is symptomatic or tested positive for COVID-19. This information will allow the College to reduce the risk of any potential spread of COVID-19 and promote self-quarantine/self-isolation and social distancing best practices. The College will not release personal information about students, employees, contractors/vendors, or visitors utilizing this form in accordance with HIPAA and FERPA .
Anyone experiencing symptoms (fever, subjective fever, new or worsening cough, shortness of breath, sore throat, muscle aches, chills, fatigue, stuffy nose, nausea, vomiting, diarrhea, or decreased sense of smell or taste) or who has been in direct contact with a person who has tested positive should be tested for COVID-19.
If you are experiencing symptoms and are awaiting testing:
- Stay home if you are sick,
- Self-quarantine/self-isolate,
- Contact your primary care provider
